A herniated disc happens when disc material pushes out and irritates or presses on a nearby nerve. Some herniated discs cause little or no pain. Others can cause strong arm or leg symptoms.

Treatment depends on the nerve involved, the amount of weakness or pain, how long symptoms have lasted, and whether the problem is improving.

At a Glance

  • A herniated disc can press on a nearby nerve.
  • Many cases improve without surgery.
  • Weakness, severe nerve pain, or symptoms that do not improve may lead to a surgical discussion.

What does a herniated disc feel like?

A lumbar herniated disc can cause sciatica, with pain that travels from the back or buttock into the leg and sometimes the foot. A cervical herniated disc can cause pain that travels into the shoulder, arm, or hand. Numbness, tingling, and weakness can occur when a nerve is compressed (radiculopathy).

Some disc herniations cause only back or neck pain, and some cause no symptoms at all.

How is a herniated disc diagnosed?

The exam checks strength, sensation, reflexes, and movements that reproduce the symptoms. MRI can show the discs, nerves, and spinal cord. X-rays can help rule out other causes but do not show a disc herniation well.

The diagnosis is strongest when the herniated disc on imaging matches the nerve pattern on the exam.

Can a herniated disc improve without surgery?

Many people improve with time, activity changes, medication, and guided exercise or physical therapy. If another type of non-surgical care is appropriate, it can be coordinated with the proper provider.

Urgent medical attention is needed for new bowel or bladder loss, saddle numbness, or major and worsening weakness.

When might surgery be considered?

Surgery may come up when nerve pain remains severe, weakness is progressing, or non-surgical care has not helped enough. In the lower back, microdiscectomy or another decompression may remove disc material pressing on a nerve. In the neck, ACDF or cervical disc replacement may be options for selected patients.

Fusion is used only when there is another reason for stability or the condition calls for it.

Common Questions

Can a herniated disc heal without surgery?

Yes. Many herniated discs improve with time and non-surgical care.

Can a herniated disc cause numbness or weakness?

Yes. A disc pressing on a nerve can cause pain, tingling, numbness, or weakness in the area served by that nerve.

What surgery is used for a lumbar herniated disc?

Microdiscectomy or another decompression can be used when a disc fragment is pressing on a lumbar nerve.

When is a herniated disc an emergency?

New loss of bowel or bladder control, saddle numbness, or rapidly worsening major weakness requires urgent medical evaluation.
